- Federal Withholding
If you wish to change the number of your Federal withholding allowances, please complete a new Form W-4 and submit it to Payroll.
- New York State Withholding
If you wish to change the number of your New York State withholding allowances, please complete a new Form IT-2104 and submit it to Payroll.
- Direct Deposit
To sign up for or change your direct deposit information, please complete a new Direct Deposit Authorization Form and submit it to Payroll.
- 403b Deductions
To sign up for, suspend, or make changes to your 403b deductions, please complete a new Salary Reduction Agreement and submit the form directly to the Omni Group. Once they process the form, they will notify our Payroll office of the appropriate changes. You can access the most current versions of these forms directly from Omni's website, www.omni403b.com
